Downloading and Installing Mozzaz Care Pro

  1. Go to the Google Play Store on the device and search for "Mozzaz Care Pro". Click on it in the results and then press Install.

  1. Enter the username and password for your account, then tap Login.

  2. The first time that you login you will see the screen below. Choose your account (the account you intend to use on the device) from the list.

Follow these steps if you see the "Missing linked page" message after logging in.

  1. Tap on Edit Mode in the lower left corner of the screen.

This project was initiated by Centerstone of Tennessee, and funded through a grant by Substance Abuse and Mental Health Administration. Centerstone of Tennessee, through partnership with the Tennessee Department of Mental Health and Substance Abuse Services (TDMHSAS), Tennessee Suicide Prevention Network (TSPN) and Centerstone Research Institute (CRI), seeks to provide training, support and services that promote a reduction in suicidal ideation and attempts in Tennessee. Through this collaboration, it is the goal to drive suicides to zero.

We are excited to offer you this application, an innovative smartphone program to provide you ongoing support and resources, help us stay connected and provide you with support when it matters most.

This program regularly checks in with you to see how you’re feeling, tracks sleep and other daily activities so that we know how best to assist you.

The Tennessee Suicide Prevention Network (TSPN) is the statewide public-private organization responsible for implementing the Tennessee Strategy for Suicide Prevention as defined by the National Strategy for Suicide Prevention. TSPN is a grass-roots association which includes counselors, mental health professionals, physicians, clergy, journalists, social workers, and law enforcement personnel, as well as survivors of suicide and suicide attempts. TSPN works across the state to eliminate the stigma of suicide and educate communities about the warning signs of suicide, with the ultimate intention of reducing suicide rates in the state of Tennessee.

Tennessee Department of Mental Health and Substance Abuse Services

The Tennessee Department of Mental Health and Substance Abuse Services is the State’s public mental health and substance abuse authority. Our responsibilities include assisting individuals to secure treatment and recovery services for serious mental illness, serious emotional disturbances, and substance abuse addiction. We also provide system planning; monitor, license, evaluate, and set policy and quality standards; collaborate with other state and community agencies; and work to educate the community about mental health and substance abuse services.

How Mozzaz Care PRO Helps

An OT working in a hospital rehab clinic manages their case load through the clinic’s Electronic Health Record (EHR) system including client in-take, appointments, billing, assessments, and care planning. The hospital uses Mozzaz Care PRO to deliver care plans on tablets for the patients and allow efficient remote monitoring and adjusting through the Mozzaz Portal. Care plans are created through the EHR and seamlessly delivered to Mozzaz Care PRO on a tablet device for the patient to use to enhance his/her own therapy regime.

Patients are regularly discharged from regional mental health facilities to a residential setting, but still require on-going care and support in order to lead a successful life. Ensuring that these plans of care are being followed and outcomes are being met is crucial to success. The State facility will then use Mozzaz Care PRO for an ease of transition of care as well as remote monitoring of patient engagement. Since the patient has been using Mozzaz Care for therapy and communication, their content and care plan will already be accessible to them as they transition from one facility to another and then eventually into the community, ensuring the continuity of care and remote patient monitoring.
